I wear a bathing suit and if I need a little more insulation a polyester or similar tight fitting shirt. Some divers I've seen wear a dive skin of varying thickness.

The most comfortable and practical item to wear under a wet suit is a  jammer style competitive tank suit. uts form fitting so it does not bunch up and leave creases or create air pockets that might lead to suit squeeze.

I wear some compression shorts to keep everything in place, and for a shirt I usually wear a tight fitting poly shirt. The compression shorts make all the difference in the world!
