Never have, I have almost ran out of air due to a leaky valve, but my dive buddy was right there and we knew what to do.

Thankfully, no. There was a guy on my dive boat who got bent. He was a pilot and had tied one on the night before. Drinking and diving just don't go together!! Happy diving

No but may have been close. 230' on air. USS Monitor dive with Jacques Cousteau's team. Insane. Definitely all narced.
